About
Julie Paul testified for the Commonwealth about treating Lindsay Clancy from November 20 through November 30, 2022. On Day 10, the psychiatric mental health nurse practitioner described Clancy's reports of anxiety and insomnia after an initially happy postpartum period. Paul said Clancy denied thoughts of harming herself or her children and reported no hallucinations during the first telephone assessment. Paul identified no delusions but believed she had not specifically asked about them.
Paul described Clancy as goal-directed and linear at the in-person intake. She reported an anxiety screening score of 21 out of 21 and an Edinburgh scale score of 23 out of 30, with a negative suicidality item, while emphasizing that screening was not diagnostic. She explained starting Prozac with Ativan support, then stopping Prozac after reported intolerance and prescribing a one-time Ambien dose followed by mirtazapine with Klonopin. Later communications addressed sleep, a dose increase and disorientation that Paul thought might reflect sedation.
After a reported panic attack, Paul recommended Ativan, a run and partial hospitalization. She said Clancy considered the program logistically unworkable. Paul transferred care to Rebecca Jollotta as she prepared to leave the clinic.
On Day 11, defense counsel Kevin Reddington focused on access to care rather than challenging Paul's clinical decisions. Paul confirmed that Clancy's mother-in-law had sought her help through a mutual connection and that Clancy had her cellphone number because Paul called her on a day off. At the defense's request, South Shore Health records were admitted without objection as Exhibit 222. The Commonwealth asked no further questions, and Paul was excused.
